MANA 2021 - Midwifery Traditions and Transformations

This summer has been one of many transformations as we glean from traditions and want to bring more online learning opportunities to midwives near and far. We have had a couple of online workshops and just completed an in-person regional conference in SC hosted by Carolina Birth Junkies.

As we honor those who have come before us and work to support each other in the present moment, we are happy to present this 2 day online conference featuring MANA board members Sarah Foster, LM, CPM MANA VP and Yesenia Guzman, LM, CPM MANA Director of Professional Development as well as Sarah Hinton, CPM, LM, BSM, Dr. Sarah Buckley, Toni Harman of Microbiome online courses, Holistic Dentist Dr. Eric Kempter and Dr. Virginia Cunningham, doctor of veterinary medicine. The Critical Importance of ‘Seeding and Feeding’ a Baby’s Microbiome for Lifelong Health

A graduate of the London Film School, Toni Harman has produced and directed a number of internationally distributed documentary films including MICROBIRTH, about the origins of the human microbiome, FREEDOM FOR BIRTH, about the struggle for women's rights in childbirth and DOULA!, about the work of birth and postnatal doulas. Toni’s extensive research for MICROBIRTH led to her co-authoring the books THE MICROBIRTH EFFECT and YOUR BABY’s MICROBIOME. Toni is also a professional speaker at international conferences about the science of the microbiome and epigenetics. As Educational Leader of the MICROBIOME COURSES online school, with over 37,000 enrolled students, Toni combines knowledge gained from researching the subject for ten years with her extensive experience of running educational one-day and multiple-day workshops for adult learners. 

1. Define the human microbiome.

2. Describe the ‘main seeding event’ for founding the baby’s microbiome during vaginal birth.

Abstract:

MICROBIRTH filmmaker and author Toni Harman explains the microscopic events happening during physiological birth and breastfeeding. When a baby is born, the infant acquires bacteria from the mother’s birth canal, from contact with the mother’s fecal matter, from skin-to-skin contact, from other environmental exposures and from breast milk. The special sugars in human breast milk then feed the bacteria that have come from the mother to quickly colonise the baby’s gut, thus ensuring the optimal training of the infant immune system. However, C-section, antibiotics and formula feeding can impact these natural biological processes with ramifications for a child’s lifelong health.

Bringing the Midwifery Model of Care to Everything You Do: How to Infuse Your Practice With Compassion and Excellence When Using Pharmacologic Therapies

Ensuring that families are treated with respect, dignity, and kindness during some of life’s most vulnerable moments is my calling, my passion, my purpose, and my greatest joy. Midwifery captured my heart and my mind in 2010 and I have been attending births ever since. The journey has taken me to places I never dreamed, and I currently work in my home state of Kansas as well as internationally in midwifery education, research, and clinical practice. I am also an animal lover, a competitive athlete, and acro yoga enthusiast.

1. Apply the basic tenets of informed consent to client care in clinical and emergency settings. Identify appropriate level of informed consent for a given situation and implement communication methods that facilitate appropriate shared decision making with the client. Avoid behaviors that reduce the ability of clients to have quality decision making opportunities.

2. Identify and implement appropriate techniques that will reduce pain and ease the client’s experience with painful procedures. Provide appropriate comfort measures to clients.

3. Describe and implement basic safety checks to prevent administration mistakes - employ the six “rights” of drug administration to ensure appropriate administration takes place.

Abstract:

Midwives frequently consider pharmacologic therapy as separate, or least very different, from the other aspects of care we provide, but in this session, you will learn practical ways to bring the midwifery model of care to your use of pharmaceuticals in your practice. Including: How to deeply embed informed consent practices into your interactions with clients. Techniques for providing care that is gentle, holistic, and culturally appropriate. How to get more information when you need it. How to avoid making potentially deadly medication errors. How to appropriately store and organize your medications and reduce your on-call stress related to keeping your supplies and equipment fully stocked and in working order. You will walk away with so many resources and practical tools for bringing excellent and compassionate care to your clients as you see them though the many challenging situations you will face together.

Incorporating the Human-Animal Bond Into Midwifery and How Not to Get Bitten

Dr. Cunningham is a wife and mother of three. She is a small animal veterinarian who currently performs locum work in general medicine and emergency. She graduated from Virginia Tech in 2000 with a degree in Animal and Poultry Science and from Ross University School of Veterinary Medicine in 2005. She became interested in midwifery after the birth of her daughter. She has found many pearls of wisdom in the midwifery model of care that she has been able to translate to her practice of veterinary medicine.

1. Explain the importance of the Human-Animal Bond.

2. Describe how to recognize the “Ladder of Aggression”.

3. List strategies to limit the animal bite risk to the birthing team during a home visit and birth while utilizing the Human-Animal Bond.

Abstract:

Homebirths involve every family member the birthing person would like to have present. Pets are considered a member of the family and midwives need to know how to safely work with and around them. This presentation discusses how to use the relationship between the family and the pet during labor, recognize when the pet is becoming stressed by the events of labor, and develop strategies to reduce the risk of being bitten. This presentation looks at the newest research on The Human Animal Bond, The Ladder of Aggression, what we have discovered about oxytocin, mental health, and pet ownership during the Covid pandemic. Anyone who works around pets during a homebirth will benefit from an understanding of the signs of stress in dogs and will reduce their chances of being bitten while in the home.

Oxytocin and the Transformation of Birth

Dr. Sarah Buckley is trained as a GP/family physician with qualifications in GP-obstetrics. She has been writing and lecturing to childbirth professionals and parents since 1997 and is the author of the best-selling book Gentle Birth, Gentle Mothering. Sarah has a special interest in the hormones of physiological labour and birth and the impacts of interventions. In 2015 she completed an extensive report on this topic, Hormonal Physiology of Childbearing, published with Childbirth Connection (US). She is currently a PhD candidate at the University of Queensland, researching oxytocin in labour and birth. She has co-authored several papers on oxytocin in labour, birth and breastfeeding. Sarah is also the mother of four children, all born at home and now in their teenage years and beyond. She lives on the semi-rural outskirts of Brisbane. For more, see www.sarahbuckley.com

Abstract:

Oxytocin is a major hormone of labour and birth. As well as facilitating the rhythmic contractions of labour in all mammals, oxytocin also acts within the maternal brain at this time, with calming, connecting and pain-relieving effects. Oxytocin activates maternal behaviour in all mammals including by activating brain reward and pleasure centres through the oxytocin peaks of labour and birth. The oxytocin system in labour and birth can be disrupted by obstetric interventions. This presentation draws from current knowledge, human and animal research, and biological understandings to discuss possible positive and negative impacts of synthetic oxytocin, epidurals, and caesareans on maternal and fetal oxytocin systems. Strategies to fill possible ‘hormonal gaps’ are also discussed.

Dr. Eric Kempter is a licensed dental practitioner in the state of North Carolina. He is the Director of Clinical Operations at Kempter Holistic Dentistry. Dr. Kempter, along with his brother Dr. John Kempter, provide a whole-body holistic approach toward dental health. Dr. Eric Kempter is one of few providers of lip and tongue tie releases in North Carolina. Dr. Kempter has helped aid numerous mothers in achieving more comfortable, productive breast feeding for both mothers and their infants.

Navigating Intergenerational Apprenticeships

Yesenia (she/her) graduated from Bastyr University in 2013 with a Master of Science in Midwifery. Bastyr University, a natural medicine school, offers the only regionally accredited Master’s degree for direct- entry midwifery in the nation. She balances this scientific education with ancestral and intuitive knowledge and a respect for informed choice. Currently based in San Antonio, Yesenia is opening her own homebirth practice, Alumbramiento Midwifery, and is active in American Indians of Texas as a Family Health and Wellness Specialist. In addition, she serves on the Midwives Alliance of North America Board as Director of Professional Development. Midwifery is a vehicle to bring positive change into the world. In order for that to happen, we need midwives who are as diverse and the people they hope to serve. In precepting midwifery students, she has strived to be compassionate and exacting to graduate capable midwives who will be a credit to the communities they serve.   

In her free time, you may find her walking with her pup, Yoli, reading, doing crossword puzzles or enjoying good food and company. If she is off-call, chances are she is traveling the world to gain different perspectives on life.

1. Define generational conflict. Why do we separate out each generation the way we do and how does the culture at the time they were being raised influence their way of viewing the world.

2. Explain how interactions between different generations can cause conflict and how it might manifest in the apprenticeship.

3.Discuss solutions to resolving those conflicts and being able to continue working together with the goal of training fully qualified entry level midwives without burning out as preceptors.

Abstract:

Generational differences are bound to come into play in apprenticeships. In this workshop we will talk about the differences between generations and their expectations as well as how preceptors can navigate the possible conflicts inherent in those differences with the goal of training fully competent entry level midwives while not burning ourselves out as preceptors.

1.Describe the causal links between sleep loss and preterm birth, prenatal and postpartum depression.

2. Describe the causal links between maternal sleep position on disordered maternal breathing and stillbirth.

3. Explain how to counsel clients of best sleep practices and how be alert to the insomnia/depression dyad.

Abstract:

This presentation will cover the importance of sleep in pregnancy, average sleep in each trimester and the effects of lower quality and duration of sleep as well as sleep position. Lower sleep quality and shorter duration of sleep has been linked with preterm birth, prenatal and postpartum depression, while supine sleep position in third trimester has been linked with stillbirth. We will explore what the research says about causality and pathways as well as how we can educate our clients to modify sleep behavior and potentially limit their risk of these outcomes.

Our thyroid is one of our master glands when it comes to hormone production and proliferation. The demands of pregnancy cause some changes which are normal. We will briefly cover the basics then go into thyroid dysfunction: recognizing signs, ordering and interpreting lab results, effects on pregnancy and baby. Then we will cover how to support with herbs and nutrition and when to refer for further treatment/co-care.
