Breastfeeding improvement following tongue-tie and lip-tie release: A prospective cohort study

Bobby Ghaheri, MD is a board certified ear, nose and throat specialist with The Oregon Clinic in Portland, OR. His interest in treating children with tongue and lip-tie stems from his ardent support of breastfeeding and was furthered by his personal experiences, as his youngest child benefited from treatment for it. He enjoys working with children and has an interest in traditional and non-traditional approaches to pediatric pain control. To communicate with him, feel free to email him at [email protected] or follow him on Twitter at @DrGhaheri.

Objective 1: Describe the impact of tongue tie release on GERD.

Objective 2:Describe the impact of tongue tie release on maternal breastfeeding self-efficacy.

Objective 3: Describe the similarities in improvement in children with both anterior tongue tie and posterior tongue tie.

Abstract:

Numerous symptoms may arise which prevent mother-infant dyads from maintaining desired breastfeeding intervals. Investigations into treatments which positively influence breastfeeding outcomes allow for improved patient counseling for treatment decisions to optimize breastfeeding quality.

Surgical release of tongue/lip-tie results in significant improvement in breastfeeding outcomes. Improvements occur early (1-week postoperatively) and continue to improve through 1-month postoperatively. Improvements were demonstrated in both infants with classic anterior tongue-tie and less obvious posterior tongue-tie. This study identifies a previously under-recognized patient population that may benefit from surgical intervention if abnormal breastfeeding symptoms exist.

People recognize her as an expert on infant sucking issues caused by various structural problems like torticollis, plagiocephaly, brachycephaly and tissue shock-trauma. She invented the Hazelbaker™ FingerFeeder and the Infant Breastfeeding CranioSacral Protocol™ to assist in the resolution of this type of infant sucking dysfunction.

Objective 1: Describe the clinical ramifications of new histological findings about the lingual frenum.
Objective 2: Discuss the manner in which low tongue posture affects future postural development and airway integrity.
Objective 3: Describe the need for evidence-based clinical decision making when it comes to revision wound healing.
Objective 4: Discuss the normal morphology of the infant maxillary frenum.
Objective 5: Describe the incidence of posterior vs. anterior tongue-tie.

USA Alison K. Hazelbaker, PhD, IBCLC, FILCA, CST, RCST, PPNE
Abstract:

This presentation examines several influential recent studies that will change the way we think about tongue-tie; the manner in which we examine, treat and perform therapy on babies. From the overall perspective of development, Dr. Hazelbaker looks at histology, speech development, posture, wound healing, lasers and airway integrity, citing the most exciting studies on these subjects, explaining how each will impact our philosophical, intellectual and clinical involvement with study findings. Further, Dr. Hazelbaker makes suggestions of how these various studies should guide our thinking about when and how to assess and treat tongue-tied infants.

Will It Hurt? Frenotomy Aftercare Strategies to Optimize Healing Outcomes for the Newborn
U.S.A. Melissa Cole, MS, IBCLC

Melissa Cole is a board-certified lactation consultant, neonatal oral-motor assessment professional and clinical herbalist in private practice. Melissa is passionate about providing comprehensive, holistic lactation support and improving the level of clinical lactation skills for health professional. She enjoys teaching, researching and writing about wellness and lactation-related topics. Her bachelor’s degree is in maternal/child health and lactation and her master’s degree is in therapeutic herbalism. Before pursuing her current path, Melissa’s background was in education and cultural arts, which has served her well in her work as a lactation consultant and healthcare educator. She loves living, working and playing in the beautiful Pacific Northwest with her 3 children.

Objective 1: Describe why minimizing neonatal pain is important
Objective 2:Describe the impact of stress on wound healing
Objective 3:Identify some pain relief strategies for post frenotomy care

U.S.A. Melissa Cole, MS, IBCLC
Abstract:

There are a few emerging theories and clinical practice ideas when it comes to frenotomy aftercare for the newborn. Clinicians and families sometimes struggle with the idea of how to balance efficient post-frenotomy care while minimizing stress and discomfort of the newborn. While the evidence strongly points to the benefit of frenotomy for tongue-tied neonates, there is little published evidence regarding best aftercare practices. During this presentation we will explore the neonatal experience of pain along with pain management and oral healing strategies. We will also discuss the impact of stress on wound healing and focus on ways to help keep babies calm and happy through the recovery process. For families coping with a tongue-tied baby, it is often a physical and emotional roller coaster ride. This presentation will help provide a well-rounded view of frenotomy aftercare issues as well as clinical, care strategies to optimize healing and feeding for the newborn.

Aerophagia Induced Reflux Associated With Lip and Tongue Tie in Breastfeeding Infants
USA Dr. Scott Siegel, MD, DDS, FACS, FICS, FAAP

Dr. Siegel is a pioneering Tongue Tie surgeon, mentored through medical school, residency and in private by practice by the late Elizabeth Coryllos, MD. He is a dual degree MD, DDS Oral and Maxillofacial Surgeon, lectures internationally and publishes on the topic of Reflux associated with Lip and Tongue Ties.

Objective 1: Discuss pathophysiology of infantile reflux vs Aerophagia Induced Reflux (AIR)
Objective 2:Describe role of lip/tongue tie in aerophagia induced reflux (AIR)
Objective 3: Explain treatment with frenotomy/frenectomy for AIR.

USA Dr. Scott Siegel, MD, DDS, FACS, FICS, FAAP
Abstract:

This study aims to show a correlation between aerophagia in infants with tethered oral tissues (TOT’s) consisting of ankyloglossia and shortened maxillary labial frenula (tongue and lip-tie) and reflux. Currently there is little to no data on aerophagia as a possible cause of reflux in infants. Infants with tongue and possibly lip tie often have a poor latch in which there is often an inadequate seal around the breast and inefficient transfer of milk. As a result many of these infants swallow air during breastfeeding. Many of these infants suffer from symptoms of reflux. This study and results to demonstrate a correlation of ankyloglossia and shortened maxillary labial frenula with aerophagia and reflux.Treatment of these infants with a relatively simple frentomy procedure may reduce or eliminate reflux in this population.

Objective 2:Identify common signs/symptoms of tethered oral tissues related to feeding and speech function.

Objective 3: Advocate for intervention via appropriate referrals to designated professionals treating tethered oral tissues for feeding and speech function.

Abstract:

This presentation is an introduction to tethered oral tissues and their impact on oral function beyond breastfeeding. Participants will learn about the oral skills necessary for proper speech and solid feeding and how tethered oral tissues alter this optimal function. Participants will learn how to effectively communicate with patients, families, and other medical professionals on this topic and make appropriate referrals with recognition of signs and symptoms affecting oral function.
